New Patients

Your first visit to Dr. Donald Kuzyk's office involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You'll find all the practical information you need, such as maps and directions to our office. You can also save some time by printing out and completing the patients forms in advance of your appointment.

Mission Statement
"People are to be treated like humans not just another number. We will take the time to listen to what the patient has to say and to speak truthfully to them. Whether it is the Patient or the Doctor, NO BODY LIKES TO WAIT, so that is why (with a few exceptions) the patient will see Dr. Kuzyk with in 15 minutes their appointed time.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this Web site. Driving Directions

The Simi Valley Office is across from the Simi Hospital's emergency room. On the front of the building you will see the Medicine Shoppe Pharmacy. Park in the back of the building and use the back entrance. We are the first door on the left when you come in from the back or the last door on the right when you come in from the front of the building.

The Newbury Park Office is located in the Times Square Shopping Center across from The Alamo Restaurant. It is behind Baskin Robins Ice Cream and Judy's Donuts and between Stella's Restaurant and IHOP.
