Office Hours
Our office is open from 9:00 am to 5:30 pm Monday thru Friday and one Saturday a month from 9:00 am to 12:30 pm.

The office will be closed from 1:00 pm - 3:00 pm for lunch

Appointments
You can request an appointment by filing out this form and clicking send. We will call you to make/confirm your appointment. If you need an immediate appointment, please call (805) 499-3287.

Insurance
Aetna, Blue Cross, Blue Shield, Cigna, Medicare, Medi-Cal, Motion Picture, Regal Medical Group, Simi Valley Hospital EPO, United Health Care, and many others. While Dr. Kuzyk's office accepts most insurance plans it is the final responsibility of the patient to know the details of their coverage, if a prior authorization is needed to see Dr. Kuzyk, and copayment / deductible information.

Payment Options
We accept assignment on most insurance claims which means you will be responsible for your deductible, co-payment or share of cost. Personal checks and cash payments are welcome. We accept Mastercard, Visa, and Discover Cards.
